Plantar Fasciitis: Do Night Splints and Orthotics Help?

The first few steps out of bed can be the worst part of plantar heel pain. The heel may loosen as you move, only to become sore again after a long period of standing, walking, or sitting. That pattern is commonly associated with plantar fasciitis, but not every painful heel has the same cause.

Night splints and foot orthotics are frequently recommended—and frequently marketed as complete solutions. The evidence supports a more measured answer: both can be useful for selected people, but neither replaces a broader plan that restores mobility, builds capacity, and adjusts the activities provoking the pain.

Bottom line: Night splints are most relevant when first-step morning pain is persistent. Orthotics can support comfort and activity, but current guidance says they should be combined with stretching, progressive loading, and other appropriate care—not used as the only treatment.

What plantar fasciitis usually feels like

The plantar fascia is a strong band of connective tissue running from the heel toward the toes. It helps support the arch and manage load as the foot accepts weight and pushes off the ground.

Typical plantar fasciitis features include:

  • pain near the inside-bottom portion of the heel;
  • pain with the first steps after sleep or another period of rest;
  • symptoms after a recent increase in walking, running, standing, or other weight-bearing activity; and
  • tenderness close to the plantar fascia’s attachment at the heel.

Despite the familiar name, the condition is not always a simple inflammatory problem. The tissue can show a mixture of irritability and longer-term degenerative change. That is one reason a single passive product rarely resolves it on its own.

Heel pain can also arise from a calcaneal stress injury, heel-pad irritation, nerve irritation, inflammatory disease, or pain referred from elsewhere. An assessment matters when the pattern is unusual or recovery is not progressing.

What the evidence says about night splints

A night splint keeps the ankle from resting in a fully pointed position while you sleep. Depending on its design, it also maintains a gentle stretch through the calf and plantar fascia.

The 2023 heel-pain clinical practice guideline recommends a one- to three-month night-splint program for people who consistently have pain with their first step in the morning. This is a targeted recommendation: a night splint makes the most sense when morning start-up pain is a prominent, repeatable symptom.

It is not a guarantee, and comfort matters. A splint that causes numbness, skin pressure, cramping, or worse sleep is not helping simply because it creates more stretch. Follow the product instructions, begin with the least aggressive comfortable position, and ask a clinician for help if you cannot tolerate it.

What the evidence says about foot orthotics

Foot orthotics can cushion the heel, support the arch, or change how load is distributed through the foot. Prefabricated and custom devices are both used for plantar heel pain.

The same 2023 guideline makes two important distinctions:

  1. Orthotics should not be used as an isolated treatment for short-term pain relief.
  2. Orthotics may help reduce pain and improve function when combined with other treatments.

Across the evidence reviewed for the guideline, custom devices were not consistently superior to prefabricated orthotics. The best device is therefore not automatically the most expensive one. It is the one that fits your footwear, feels tolerable, matches your foot and activity needs, and supports a plan that also addresses mobility and loading.

An orthotic that creates new pressure points, numbness, or increasing pain should be reassessed rather than “broken in” through significant symptoms.

The treatments with the clearest role

The strongest plan is usually multimodal. The clinical practice guideline supports:

  • Plantar fascia-specific stretching and calf stretching for short- and longer-term improvements in pain and function.
  • Progressive strengthening and loading to improve the foot and lower leg’s ability to tolerate daily demands.
  • Manual therapy when relevant joint or soft-tissue restrictions are present.
  • Foot taping alongside other treatment for short-term improvements in pain and function.
  • Activity and footwear changes that reduce a sudden load spike without imposing unnecessary total rest.

The aim is not to avoid using the foot forever. It is to reduce excessive irritation while gradually restoring what the tissue needs to handle.

A practical decision guide

Consider a night splint when:

  • first-step morning pain is one of your most consistent symptoms;
  • the foot feels especially tight after rest; and
  • you can wear the device without disrupted sleep, pressure, or neurological symptoms.

Consider an orthotic when:

  • cushioning or arch support makes walking more comfortable;
  • your current footwear offers little support or shock absorption;
  • taping provides short-term relief and a clinician thinks a similar support strategy may be useful; or
  • you need an adjunct that helps you stay active while following a rehabilitation plan.

Do not rely on either product alone when:

  • activity tolerance keeps declining;
  • symptoms are worsening despite reducing the provoking load;
  • you have not addressed calf/foot mobility and strength; or
  • the diagnosis has never been assessed and the symptom pattern is atypical.

When heel pain needs an assessment

Seek assessment from a physiotherapist, physician, or other qualified clinician when:

  • pain began after a fall, impact, or sharp increase in training;
  • you cannot bear weight normally;
  • pain is severe at rest or wakes you consistently at night;
  • there is marked swelling, redness, warmth, fever, numbness, or tingling;
  • pain is spreading or associated with back/leg neurological symptoms; or
  • several weeks of sensible load adjustment and rehabilitation are not helping.

Imaging is not routinely needed for a typical presentation at the outset. It becomes more relevant when the diagnosis is uncertain, another condition is suspected, or conservative care has not produced the expected progress.

Frequently asked questions

Will a night splint cure plantar fasciitis?
No. A night splint may reduce morning start-up pain for some people, but it does not replace progressive exercise, load management, and treatment of relevant mobility or strength deficits.

Are custom orthotics better than store-bought inserts?
Not automatically. Current evidence does not show consistent superiority of custom over prefabricated orthotics for plantar fasciitis. Fit, comfort, footwear compatibility, and the rest of the treatment plan are more important than price alone.

Should I stop walking until the pain is gone?
Complete rest is rarely the long-term answer. Reduce or modify the activities that clearly flare symptoms, then rebuild walking or sport exposure gradually. A clinician can help set an appropriate progression if normal walking remains difficult.

Is a heel spur causing the pain?
Heel spurs can appear on imaging in people with and without symptoms. Their presence does not by itself prove the source of pain or determine treatment.
